As cancer survival rates continue to improve, nephron-sparing surgery is being increasingly considered as a viable treatment option for renal cell carcinoma, especially for patients with localised tumours that are smaller than 4 cm. Because renal cryoablation is a minimally invasive surgical procedure with excellent oncological outcomes, many urologists are turning to it to treat the tumours that are not amenable to surgery. Despite this, the reported rates of local recurrence and progression after cryotherapy have varied widely across studies. We aimed to systematically review the published literature on renal cryotherapy and to provide an overview of its reported outcomes and shortcomings. A literature search of PubMed and MEDLINE was performed from inception to July 2018 for studies reporting on renal cryotherapy. A total of 10 studies with 971 patients were included in the final review. The reported local recurrence rates after cryotherapy ranged from 0 to 19% with a median of 6.5% (IQR: 4-10%). Renal function preservation was noted in 99% of the patients and the reported incidence of a second nephrectomy ranged from 0 to 17%. Two studies reported the use of pre-cryotherapy embolisation to decrease the tumour size and thus, the cryoablation size, with similar oncological outcomes. The development of local recurrence after renal cryotherapy seems to be related to pre-treatment size and margin status. Further studies are needed to establish whether the risk of local recurrence can be further reduced with embolisation or different cryosurgical probes.Charles F. Wilson
